Direct answer. Insurance businesses in the UK and India grow with AI, automation and custom software when one claim file shows who was notified and when, which cost or unpaid part of the bill is still open, why that line is still open, and the name of the person who accepted the next step, before more money runs or a grievance is closed only to reopen. Cogitix builds that file beside the claims system the business already uses. This article does not invent a savings percentage, and it is not insurance, legal or medical advice.
The examples are patterns, not client stories. Cogitix is an AI agency that builds software for clients worldwide. It does not decide a claim.
What file is checked before a claim cost keeps running?
A UK motor claim and an Indian health claim both need a record a later reviewer can reconstruct. It is still split across a first-notice call, a hire invoice, a hospital bill, a chatbot transcript, and a complaints inbox the claims desk cannot open. Before the next cost runs, or before a grievance is marked closed, the file stores who was told and the date, which head or part of the bill is still open, why that line is open, and who accepted pay, query, wait or stop. Waiting is an honest status.
Why does a silent claim file hurt?
On 3 November 2026 Bryony Garlick at Insurance Business reported Chris Clapp of All-Rates. A taxi, a motorcycle, or a late report is often the first sign credit hire will climb. Those vehicles fall outside large insurers' arrangements and the ABI General Terms of Agreement far more often, at £300 or £400 a day and upwards. A fault notice with no hire notice can leave the charge invisible. On the cases he sees, storage used to attach to roughly 15 to 20% of credit hire claims at about £35 a day, and he now sees it on about 35%, at £50 and up to £100. That is his experience, not a survey. Outside the agreement he described £200 to £800 or £900 a day, and said missing evidence would settle 90 to 95% of claims stuck at that hire-rate stalemate.
On 20 August 2026 Iris Hakaj at Insurance Times reported a Huntswood study of 1,025 people. Nine per cent had claimed in a year, and 11% called the experience poor or very poor. Car insurance was 49% of claims, home or property 26%, pet 10% and travel 9%. Of the poor experiences, 39% named an unnecessary delay, 29% a slow settlement, and 23% said chatbots and self-service had hindered the claim. Twenty-five per cent had switched because of the process, and 30% planned to switch at renewal. Siddharth Parashar called complaints "one of the clearest indicators" of delay and friction. Huntswood handled more than 294,000 complaints for UK organisations in 2025, around 134,000 for insurers.
The Financial Conduct Authority's February 2026 insurance priorities say too many claims go badly. Firms should state the cover, respond promptly and fairly, and test outcomes where an outsourced firm or a delegated authority holds the file. With the ABI it will work on claims cost and speed. It supports AI where the firm watches the customer's result. Ghost broking remains a named concern.
A CarInsuranceUK post, labelled about a year old when opened on 5 November 2026, describes three Admiral paths after a shunt where liability was admitted: claim direct and pay the excess, recorded as not at fault; an accident management company, with liability for hire it cannot recover, recorded as notification only; or the other insurer running the repair, also notification only. The thread asks which label costs less next year. The file records the path, the status, and who pays unrecovered hire, before anyone signs.
In India the open file is often a year old. On 2 November 2026 Elets reported IRDAI's Grievance Resolution Month, launched in Hyderabad on 31 October. Citing IRDAI, about 2.21 lakh cases (about 221,000; a lakh is 100,000) were pending before Consumer Commissions and courts on 31 March 2026, roughly 72.49% for more than a year, plus around 11 lakh (about 1.1 million) before Motor Accidents Claims Tribunals. Insurers were asked to clear old cases, settle where the facts allow, comply with Ombudsman awards, and name recurring causes. Ajay Seth said success "must not be the number of cases closed alone, but the fairness, quality and sustainability of the resolutions achieved."
A handler can also lose the bill or the cover. On 3 November 2026 Zee News English quoted the health master circular: the primary insurer coordinates the unpaid balance "without causing any hassles to the policyholder." Its illustration, not an average, is a Rs 10 lakh bill against Rs 5 lakh of cover, with Rs 50,000 inadmissible on the first policy. The mistake it names is a missed notice to both insurers. On 4 November The Economic Times reported four motor layers from 15 December 2026, after National Insurance Co versus Thungala Dhana Laxmi, plus an option form and Customer Information Sheet. On 29 August The Hindu reported a 28 August meeting on disclosure at sale and the National Health Claims Exchange.
An r/indiahealthinsurance post, about two months old when opened for this article, says pre-authorisation was refused without a clear reason, the same papers were demanded twice over a month and a half on the last day of each 15-day cycle, and nobody confirmed receipt. Mail hit an automation, the portal crashed, and Bima Bharosa locked the poster out. One thread is not a rate.
Where should an insurer or broker start?
Keep the policy system, the loss adjuster and the person who may pay. Add one record for one queue: credit hire on taxis and motorcycles where the hire notice may arrive late, home and travel claims held by an outsourced team, health claims where a second policy may cover the balance, or motor sales that must carry the new option form before 15 December. Each line has a source, a date, and a status of known, open, not known or waiting. That is where digital transformation consulting should start.
How do four steps put the claim on one record?
1. Write the claim once
Store the policy, the date of loss or admission, who was told, and the heads still running: hire, storage, repair, a hospital line, a second insurer, an award. A missing notice, a missing option form, or a document nobody has marked received is stored as "not notified", "not on file", or "receipt not confirmed". Capture it with custom web development, a mobile app, or WhatsApp automation where the bill already arrives there.
2. Ask the person who knows
A draft may set the open heads beside the dates and the cover that was bought. It stays a draft until a person accepts it. Software must not admit liability, invent a hire rate, or treat a chatbot transcript as a refusal. That limit is AI agent integration, as described for autonomous agents.
3. A person accepts the next step before money moves
A person chooses pay, query, wait or stop. Software holds the next payment while notice is missing, the remainder has no second insurer, or nobody has signed. A without-prejudice payment, which Clapp described as a way to stop a running hire cost, is still that person's decision. The software records it. It does not send the money.
4. Keep one pack a person has read
The pack is the notice, the open head, the reason, and the name. A UK motor line shows whether hire and storage are already running. An Indian health line in this month's window shows the recurring cause. A motor sale toward 15 December includes the option form and the Customer Information Sheet. A later reader should find that same fact, which is also the work of answer engine optimisation and generative engine optimisation. Several desks on one history is cloud-based SaaS. A workflow other insurers will run is end-to-end product development.
What is different for UK insurers and brokers?
Store the vehicle type and the gap between the accident and the notice. A standard car inside a non-fault arrangement is a different queue from a taxi or motorcycle outside the General Terms of Agreement. For home and travel, name the outsourced firm that holds the next step. A chatbot answer is a dated transcript. Ghost broking is a separate line: whether a firm the insurer can identify sold the policy.
A slow claim meets the same human check in how to improve workflows using AI. A dispute with a lawyer meets the legal guide. A hospital admission meets the healthcare guide, and the clinical record stays with the provider.
What is different in India?
November 2026 is a dated queue. Record a cause a colleague can search: second insurer not notified, documents sent but not acknowledged, option form missing, award not complied with, disclosure missing at sale. Where the business uses the National Health Claims Exchange, that feed is a source on the file, and a person still accepts the line. From 15 December, put the motor option form on the claim before anyone argues about occupant cover. Keeping a fraud case apart from a payment is the subject of the fintech guide. Here, a grievance closure is not the claims decision until a person accepts it.
Should you buy, configure, or build?
Buy or configure when the product already stores the notification date, the open head, the reason, and the person who accepted the next step, and can hold the next payment while a status is waiting. Build when one business must hold a UK credit-hire claim and an Indian health grievance on the same history, when a WhatsApp photo or a hospital bill has to land on the claim, or when the complaints export cannot show why this case is still open. A person at the business still decides. Cogitix does not admit a claim and does not file a court reply.
What does a 90-day pilot include?
Days 1 to 15. One queue. List the claims, who may see bills and hire charges, and the rollback. Days 16 to 45. One team. The next payment, and the next status of closed, proceeds only after a person accepts the line. A message goes out only after a person sends it. Days 46 to 75. Recount lines with no notice date, no reason, a second policy marked "not known", a missing option form, or no name. This guide has no benchmark percentage. Days 76 to 90. Add a second queue on its own file, or stop.

- The early signs a credit hire claim is about to get expensive, Bryony Garlick, Insurance Business, 3 November 2026, reporting Chris Clapp of All-Rates. Taxis, motorcycles and late reports as early signs; charges of £300 or £400 a day and upwards outside large-insurer arrangements and the ABI General Terms of Agreement; hire charges that build while a fault notice has no hire notice; Clapp's observation that storage attached to roughly 15 to 20% of credit hire cases at around £35 a day and that he now sees it on about 35%, routinely at £50 and as high as £100; daily rates from £200 up to £800 or £900 outside the agreement; his statement that missing evidence would settle 90 to 95% of claims that reach the impecuniosity stalemate; and a without-prejudice payment as his way to stop a running cost. The storage shares are his experience, not a survey.
- Claims delays behind most insurance complaints, research shows, Iris Hakaj, Insurance Times, 20 August 2026, reporting Huntswood research. A study of 1,025 people; 9% had claimed in 12 months; 11% described the experience as poor or very poor; car 49%, home or property 26%, pet 10%, travel 9%; of poor experiences, 39% unnecessary delay, 29% delay in settlement, 23% chatbots and self-service hindering the claim; 25% had switched because of a poor claims process and 30% planned to switch at renewal; Siddharth Parashar on complaints as an indicator of delay, poor communication or friction; Huntswood handled more than 294,000 complaints for UK organisations in 2025, including around 134,000 for UK insurers.
- Regulatory Priorities: Insurance, Financial Conduct Authority, February 2026. Poor claims experiences; a prompt, fair and transparent response; clear cover and tested outcomes; improvement where claims are outsourced; an expanded review of oversight of outsourced claims, including delegated authority; work with the ABI and firms, under the Motor Insurance Taskforce, on claims efficiency and cost; support for AI with monitoring of consumer outcomes; and ghost broking remaining a concern.
- Not at Fault v Notification Only?, CarInsuranceUK. When opened on 5 November 2026 the thread was labelled about a year old. A parent describes three Admiral paths after a shunt in which liability was admitted: claim direct and pay the excess first, recorded as not at fault; an accident management company with no excess up front but liability for unrecovered amounts, recorded as notification only; or the other insurer managing the repairs, also notification only. The thread asks which record costs less at renewal. This article does not answer that question and did not rely on comments that were not in the page text.
- IRDAI Declares November Grievance Resolution Month for Insurers, BFSI Network, Elets, 2 November 2026, updated 3 November 2026. Launch in Hyderabad on 31 October 2026; about 2.21 lakh cases pending before Consumer Commissions and courts as of 31 March 2026, roughly 72.49% for more than a year; around 11 lakh cases reported pending before Motor Accidents Claims Tribunals; Ajay Seth on fairness, quality and sustainability rather than closures alone; the request to identify recurring causes and to comply with Ombudsman awards; and the article's statement that health insurance is where customer dissatisfaction runs highest.
- One hospitalisation bill, two health insurance policies, Sandal Khan, Zee News English, 3 November 2026. The Master Circular line that the primary insurer coordinates the balance with other insurers without hassle to the policyholder; the article's illustrative bill of Rs 10 lakh, sum insured of Rs 5 lakh and Rs 50,000 inadmissible; the need to notify both insurers inside the policy window; and attested copies when the first insurer takes the originals.
- Advice related to claim processing, r/indiahealthinsurance. When opened on 5 November 2026 the thread was labelled about two months old. Pre-authorisation denied without a clear justification; reimbursement after discharge; the same documents requested again twice over a month and a half, on the last day of each 15-day cycle; email going to an automation; a crashing upload portal; deficiency queries for documents already sent; an automated reply from the chief grievance officer; Bima Bharosa locking the poster out; and a request for written acknowledgement of receipt. One thread, not a rate. This article did not open comments that were absent from the page text, and it did not open the insurer's letters.
- Irdai mandates four-tier structure for motor insurance from Dec 15, The Economic Times, 4 November 2026. Four layers from 15 December 2026 for private cars and two-wheelers, after National Insurance Co versus Thungala Dhana Laxmi; a mandatory customer option form and Customer Information Sheet; premiums shown separately; occupant liability optional and addable to an existing third-party policy without cancellation.

What should a UK insurer or broker record before credit hire costs keep running?
Insurance Business on 3 November 2026 reported Chris Clapp of All-Rates: a taxi, a motorcycle, or a late report is often the first sign that credit hire will climb, and a fault notice with no hire notice can mean charges are building while the broker cannot see them. He described taxis and motorcycles falling outside large insurers' arrangements and the ABI General Terms of Agreement far more often, at £300 or £400 a day and upwards, and storage that he now sees on about 35% of the credit hire cases in his experience. Insurance Times on 20 August 2026 reported a Huntswood study of 1,025 people in which, of those who called the claims experience poor, 39% named an unnecessary delay. The FCA's February 2026 insurance priorities ask firms to respond promptly and fairly, and to test outcomes, including where claims are outsourced. Record the vehicle, the notice date, whether hire and storage are already running, the firm that holds the next step, and the person who accepts pay, query, wait or stop. This is a reading of those pages, not a claims instruction, and Clapp's storage shares are his experience.
What should an Indian insurer record during Grievance Resolution Month?
Elets on 2 November 2026 reported IRDAI's November 2026 Grievance Resolution Month, launched in Hyderabad on 31 October. The report cites about 2.21 lakh cases pending in Consumer Commissions and courts as of 31 March 2026, roughly 72.49% of them for more than a year, and around 11 lakh cases before Motor Accidents Claims Tribunals. Ajay Seth said success is the fairness, quality and sustainability of a resolution, not the count of closures. Zee News English on 3 November 2026 quoted the health master circular: the primary insurer coordinates the unpaid balance with any other policy, and both insurers need notice inside the policy window. The Economic Times on 4 November 2026 reported a four-tier motor structure from 15 December, with a customer option form and a Customer Information Sheet. Record who was told, the unpaid line and its reason, whether documents were acknowledged, whether the option form is on the file, and the person who accepts the next step. This is a reading of those pages, not a regulatory filing.
When is custom claim-file software a better fit than the existing claims system and a chatbot?
Custom software fits when the same business must hold a UK credit-hire claim and an Indian health grievance, when a bill sent on WhatsApp or email has to land on the claim, or when the complaints export cannot show why this case is still open. A claims system and a chatbot are enough only while one person can still name the notice, the open line and the status without opening every thread. Huntswood's study, reported by Insurance Times, found that 23% of people with a poor claims experience said chatbots and self-service had hindered the claim. Cogitix builds the record beside the policy system. It does not admit the claim, and it does not decide the grievance.
What should a 90-day insurance pilot include?
One queue, one team, a rollback, and a human checkpoint before the next payment and before any case is marked closed. Recount lines with no notification date, open heads with no reason, second policies marked not known, option forms missing, and decisions with no name. At day 90, add a second queue on its own file, or stop. Cogitix does not publish a standard savings percentage, because that number would not describe your claims.
Does Cogitix decide claims, or publish a phone number or a street address?
Cogitix does not decide claims, give insurance, legal or medical advice, or file complaints for a customer. The site publishes the email hello@cogitix.io and hours of Monday to Saturday, 9:00 am to 6:00 pm. A telephone number and a street address are not listed. Cogitix serves clients worldwide.
